The utility of whole-genome sequencing to inform epidemiologic investigations of SARS-CoV-2 clusters in acute-care hospitals

Objective:
To evaluate the utility of selective reactive whole-genome sequencing (WGS) in aiding healthcare-associated cluster investigations.
Design:Mixed-methods quality-improvement study.
Setting:Thes study was conducted across 8 acute-care facilities in an integrated health system.
Methods:We analyzed healthcare-associated coronavirus disease 2019 (COVID-19) clusters between May 2020 and July 2022 for which facility infection prevention and control (IPC) teams selectively requested reactive WGS to aid the epidemiologic investigation. WGS was performed with real-time results provided to IPC teams, including genetic relatedness of sequenced isolates. We conducted structured interviews with IPC teams on the informativeness of WGS for transmission investigation and prevention.
Results:In total, 8 IPC teams requested WGS to aid the investigation of 17 COVID-19 clusters comprising 226 cases and 116 (51%) sequenced isolates. Of these, 16 (94%) clusters had at least 1 WGS-defined transmission event. IPC teams hypothesized transmission pathways in 14 (82%) of 17 clusters and used data visualizations to characterize these pathways in 11 clusters (65%). The teams reported that in 15 clusters (88%), WGS identified a transmission pathway; the WGS-defined pathway was not one that was predicted by epidemiologic investigation in 7 clusters (41%). WGS changed the understanding of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission in 8 clusters (47%) and altered infection prevention interventions in 8 clusters (47%).
Conclusions:Selectively utilizing reactive WGS helped identify cryptic SARS-CoV-2 transmission pathways and frequently changed the understanding and response to SARS-CoV-2 outbreaks. Until WGS is widely adopted, a selective reactive WGS approach may be highly impactful in response to healthcare-associated cluster investigations.

Background: Identifying and interrupting transmission of severe acute respiratory syndrome coronavirus 2 and resulting disease (COVID-19) in acute-care settings can be challenging due to incubation period, asymptomatic infection, and prevalent community disease. To elucidate routes of infection and interrupt COVID-19 outbreaks with uncertain epidemiological chains of transmission, UPMC utilized reactive whole-genome sequencing (WGS) of viral specimens. Methods: UPMC infection prevention teams identified healthcare-associated COVID-19 clusters with uncertain transmission pathways among patients and/or healthcare personnel (HCP) in acute-care hospitals. Nasopharyngeal samples preserved in viral transport media were obtained for genetic analyses. Nucleic acids were extracted and WGS libraries were prepared by targeted enrichment or multiplex PCR methodologies. Resulting sequencing reads were aligned to the Wuhan-1 reference genome, followed by identification of single-nucleotide polymorphisms (SNPs) among the genomes and construction of a phylogenetic tree. Specimens were considered genetically similar if there were ≤2 SNP differences between viral genomes within a cluster. Results: Between May 2020 until August 2022, infection prevention teams requested WGS for 17 healthcare-associated clusters of COVID-19 involving 182 individuals across 8 UPMC facilities (median outbreak size, 9 individuals; range, 2–26). Of the 182 individuals, 36 lacked clinical specimens and 30 did not pass WGS quality-control criteria of ≥95% of the reference genome with a minimum of 10× coverage. Of the 116 sequenced genomes, 94 (81%) had virus genetically similar to ≥1 other specimen, including 87 (83.6%) of 104 patient viruses and 7 (58.3%) of 12 HCP viruses, comprising 22 clusters (Fig. 1). The remaining 22 (20.6%) specimens were genetically unrelated. In total, 16 (94.1%) of the 17 epidemiologically identified clusters had 2 or more individuals with a genetically similar virus. Also, 7 (41.1%) of these clusters had genetically similar viral genomes for every individual within each cluster. Also, 9 (52.9%) clusters contained both genetically related and unrelated specimens: 5 of these had more complex genomic profiles (including 4 clusters containing 2 distinct subclusters of ≥2 genetically related viruses) and 1 cluster contained 3 subclusters of ≥2 genetically related viruses. In the outbreak with 3 clusters, 3 SNPs separated specimens from 2 temporally proximal clusters, suggesting possible propagation between clusters (cluster B-3 in Fig. 1). Conclusions: WGS can complement traditional epidemiological investigations of healthcare-associated COVID-19 outbreaks, revealing complex transmission dynamics. Future investigations will characterize the impact of WGS on determining specific transmission pathways in acute-care facilities.

Summary
Multilingualism affects cognitive, behavioral, and neural function across the lifespan. Here, we review the neuroimaging literature on bilingualism, multilingualism, and executive functions, focusing on three multilingual groups who rely on language control to varying degrees to overcome competition from other languages: third-language learners, multilingual adults, and simultaneous interpreters. In third-language learners, changes in brain regions underlying executive functions occur during the early stages of acquiring another language. In multilingual adults, effects of language experience reflect a qualitative difference between monolingual and multilingual processing rather than cumulative effects of increased linguistic knowledge. In simultaneous interpreters, changes in gray matter volume and white matter integrity are found in areas underlying language selection and executive functions, reflecting neural efficiency due to experience with rapid translation. The implications of these findings for our understanding of multilingualism and the value of moving beyond the monolingual–bilingual dichotomy are discussed.
The impact of autocancellation of uncollected Clostridioides difficile specimens after 24 hours on reported healthcare-associated infections: A quality improvement intervention

Objective:
To assess the impact of a 24-hour autocancellation of uncollected Clostridioides difficile samples in reducing reported healthcare-associated infections (HAIs).
Design:Quality-improvement, before-and-after implementation study.
Setting:The study was conducted in 17 hospitals in Pennsylvania.
Interventions:Clostridioides difficile tests that are not collected within 24 hours are automatically canceled (“autocancel”) through the electronic health record. The intervention took place at 2 facilities (intervention period November 2021–July 2022) and subsequently at 15 additional facilities (April 2022–July 2022). Quality measures included percentage of orders canceled, C. difficile HAI rate, percent positivity of completed tests, and potential adverse outcomes of canceled or delayed testing.
Results:Of 6,101 orders, 1,090 (17.9%) were automatically canceled after not being collected for 24 hours during the intervention periods. The reported C. difficile HAI rates per 10,000 patient days did not significantly change. These rates were 8.07 in the 6-month preintervention period and 8.77 in the intervention period for facilities A and B combined (incidence rate ratio [IRR], 1.09; 95% CI, 0.88–1.34; P = .43), and were 5.23 HAIs per 10,000 patient days in the 6-month preintervention period and 5.33 in the intervention period for facilities C–Q combined (IRR, 1.02; 95% CI, 0.79–1.32; P = .87). From the preintervention to the intervention periods, the percent positivity rates of completed C. difficile tests increased by 1.1% for facilities A and B and by 1.4% for facilities C–Q. No adverse outcomes were observed.
Conclusions:The 24-hour autocancellation of uncollected C. difficile orders reduced testing but did not result in reported HAI reduction.

We analyzed efficacy of a centralized surveillance infection prevention (CSIP) program in a healthcare system on healthcare-associated infection (HAI) rates amid the coronavirus disease 2019 (COVID-19) pandemic. HAI rates were variable in CSIP and non-CSIP facilities. Central-line–associated bloodstream infection (CLABSI), C. difficile infection (CSI), and surgical-site infection (SSI) rates were negatively correlated with COVID-19 intensity in CSIP facilities.

Objective:
To develop, implement, and evaluate the effectiveness of a unique centralized surveillance infection prevention (CSIP) program.
Design:Observational quality improvement project.
Setting:An integrated academic healthcare system.
Intervention:The CSIP program comprises senior infection preventionists who are responsible for healthcare-associated infection (HAI) surveillance and reporting, allowing local infection preventionists (LIPs) a greater portion of their time to non-surveillance patient safety activities. Four CSIP team members accrued HAI responsibilities at 8 facilities.
Methods:We evaluated the effectiveness of the CSIP program using 4 measures: recovery of LIP time, efficiency of surveillance activities by LIPs and CSIP staff, surveys characterizing LIP perception of their effectiveness in HAI reduction, and nursing leaders’ perception of LIP effectiveness.
Results:The amount of time spent by LIP teams on HAI surveillance was highly variable, while CSIP time commitment and efficiency was steady. Post-CSIP implementation, 76.9% of LIPs agreed that they spend adequate time on inpatient units, compared to 15.4% pre-CSIP; LIPs also reported more time to allot to non-surveillance activities. Nursing leaders reported greater satisfaction with LIP involvement with HAI reduction practices.
Conclusion:CSIP programs are a little-reported strategy to ease burden on LIPs with reallocation of HAI surveillance. The analyses presented here will aid health systems in anticipating the benefit of CSIP programs.

Bilingual children have better Theory-of-Mind compared to monolingual children, but comparatively little research has examined whether this advantage in social cognitive ability also applies to adults. The current study investigated whether multilingual status and/or number of known languages predicts performance on a mentalizing task in a large sample of adult participants. Multilingualism was decomposed based on whether English is the first language or not. All analyses controlled for well-known predictors of mentalizing, such as gender, same-race bias, and years of English fluency. We found a U-shaped trend, such that monolinguals and multilinguals did not differ much in their mentalizing ability, but bilinguals performed worse than monolinguals. Our study builds upon past work by examining a large sample of participants, measuring a crucial aspect of adult social cognition that has previously been unexplored, controlling for several nuisance variables, and investigating whether multilingualism leads to additional benefits in mentalizing abilities beyond bilingualism.

Summary
A large body of research has contributed to a complex picture in which bilingualism is generally associated with better performance on some cognitive tasks, particularly those that are based on executive functioning, but poorer performance on measures of verbal proficiency. However, not all studies find these effects, particularly the positive effects on cognitive function. What is now clear is that the potential impact of bilingualism on children’s cognition must be explained through multifaceted examinations of relevant factors and clarification of the specific language context from which the results emerged on an individual basis. We review the evidence for cognitive performance of children in multilingual environments and evaluate those results in terms of the type of cognitive ability being assessed and the type of environment children are experiencing. We also review how early the effects of multilingualism are detected, how long these effects last, and how childhood multilingualism can lead to brain plasticity. We conclude with a brief discussion of how multilingualism impacts other areas of cognitive functioning, such as theory of mind, creativity, and problem solving.
Non-linear associations between HPA axis activity during infancy and mental health difficulties during early childhood among children in rural Pakistan

Hypothalamic pituitary adrenal (HPA) axis activity may be a mechanism linking early adversity to child mental health difficulties. However, there is a dearth of longitudinal evidence for the association between HPA axis activity and mental health among children in low-resource contexts. The goal of this study is to examine linear and curvilinear associations between HPA axis activity during infancy and mental health difficulties in early childhood among children in rural Pakistan. Participants included 104 children (46% male) from the Bachpan study, a longitudinal cohort embedded within a maternal depression trial in Pakistan. We examined the associations between hair-derived cortisol and dehydroepiandosterone (DHEA) at 12 months old and mental health difficulties, measured with the Strengths and Difficulties Questionnaire (SDQ), at 36 months old. There was a significant quadratic association between hair cortisol and SDQ scores, with results showing a U-shaped relationship (i.e., having relatively high or low cortisol predicted increased mental health difficulties). DHEA showed a quadratic association with SDQ scores with an inverted U-shaped relationship (i.e., high and low DHEA was associated with decreased mental health difficulties). Results provide evidence of longitudinal and curvilinear effects of cortisol and DHEA during infancy on mental health difficulties in early childhood.

Both bilingualism and attention contribute to the development of executive functioning (EF), with higher levels of both leading to better outcomes. The present study treats bilingualism and attention as continuous variables to investigate their impact on EF. Eighty-two 9-year-olds who were attending a French school in an anglophone community completed a flanker task. Children's progress in French represented their level of bilingualism, and attention was assessed through a standard standardized instrument. Degree of bilingualism and degree of attention were both positively related to performance, but exposure to a third language in the home did not further affect outcomes.
Are stories just as transporting when not in your native tongue?

We spend much of our time consuming stories across different types of media, often becoming deeply engaged or transported into these stories. However, there has been almost no research into whether processing a story in one’s non-native language influences our level of transportation. We analyzed three existing datasets in order to compare engagement with English-language stories for those who reported English as their first language and those who reported English as their second language. Stories were presented as text (Study 1), audio (Study 2), and short films (Study 3). Across all studies, equivalent levels of narrative transportation between language groups were found, even after accounting for age and years of English fluency. These results are in contrast to some previous proposals that emotional reactions are attenuated during non-native language processing, despite equivalent levels of comprehension. Our evidence indicates that individuals processing a narrative in their second language feel just as transported into the story as those processing the same narrative in their native language.
Association between universal gloving and healthcare-associated infections: A systematic literature review and meta-analysis

Objective:
Healthcare-associated infections (HAIs) are a significant burden on healthcare facilities. Universal gloving is a horizontal intervention to prevent transmission of pathogens that cause HAI. In this meta-analysis, we aimed to identify whether implementation of universal gloving is associated with decreased incidence of HAI in clinical settings.
Methods:A systematic literature search was conducted to find all relevant publications using search terms for universal gloving and HAIs. Pooled incidence rate ratios (IRRs) and 95% confidence intervals (CIs) were calculated using random effects models. Heterogeneity was evaluated using the Woolf test and the I2 test.
Results:In total, 8 studies were included. These studies were moderately to substantially heterogeneous (I2 = 59%) and had varied results. Stratified analyses showed a nonsignificant association between universal gloving and incidence of methicillin-resistant Staphylococcus aureus (MRSA; pooled IRR, 0.94; 95% CI, 0.79–1.11) and vancomycin-resistant enterococci (VRE; pooled IRR, 0.94; 95% CI, 0.69–1.28). Studies that implemented universal gloving alone showed a significant association with decreased incidence of HAI (IRR, 0.77; 95% CI, 0.67–0.89), but studies implementing universal gloving as part of intervention bundles showed no significant association with incidence of HAI (IRR, 0.95; 95% CI, 0.86–1.05).
Conclusions:Universal gloving may be associated with a small protective effect against HAI. Despite limited data, universal gloving may be considered in high-risk settings, such as pediatric intensive care units. Further research should be performed to determine the effects of universal gloving on a broader range of pathogens, including gram-negative pathogens.
